When creating a questionnaire you can select the answer types for questions based on how you want your partner to respond. Additionally you can request document evidence for all Answer Types.
Answer Types
Our questionnaires currently support different answer types as below:
- Yes, No or NA
- Multiple Choice (Select One)
- Multiple Choice (Select all that apply)
- Open answer
- Document
- Table Format
- Other
Note: In order to commit any change, click on the Save button.
1. Yes or No:
This option allows your partner to answer in a Yes, No or NA format. For each answer choice you can create follow-up questions. Click here to learn more on follow-up questions.
You can also select the grade for each answer choice that enables for auto-evaluation. Click here to learn more on Auto-Evaluation.
2. Multiple Choice (Select one):
This option allows you to add a list of options for your partner to choose from. Your partner can choose a single answer from the list of options. You can also add follow-up questions for each option and also select the grade for auto-evaluation. Click on the links below to learn more.
3. Multiple Choice (Select all that apply):
This option allows you to add a list of options for your partner to choose from. Your partner can select multiple options from the list. You can also add follow-up questions for each option. Click here to learn more on Follow-up Questions.
4. Open Answer:
This option allows your partner to type in an open-ended answer.
5. Document:
This option allows you to request documents as an answer to the question, from your partner. You can request multiple documents for any question. You can edit the name of the document request by hovering over and clicking on the pencil icon, as shown above.
6. Table Format:
This option allows you to receive the answers from your partner in a pre-defined Table format. Click here to learn more about Table answer type.
7. Other:
You can also set the answer type to be a numerical, percentage, date, or currency using this option.
Follow Up Questions
You can choose to have follow-up questions depending on what your partner chooses as the answer. You can create follow-up questions for the following answer types:
- Yes / No
- Multiple Choice ( Select one)
- Multiple Choice (Select all that apply)
1. To create a follow-up question, click on the follow-up icon next to the answer choice as shown above. This will create a follow-up question under the parent question.
2. You can enter your follow up question text and select answer type on the right side of the screen.
Note: You can create multiple follow-up questions for a parent question and you can also create multiple levels of follow up questions.
